Families have lost trust over maternity inquiry. An independent inquiry into ‘repeated failures’ at Leeds Teaching Hospitals NHS Trust has been announced by Wes Streeting after a BBC investigation revealed the deaths of at least 56 babies and two mothers may have been prevented. MPs are calling on the Prime Minister to intervene and appoint senior midwife Donna Ockenden to chair the investigation, citing her experience leading inquiries into Nottingham maternity services and Shrewsbury and Telford Trust. A Department of Health spokesperson stated that they are working with families to appoint a chair and agree on terms of reference for the review. Ockenden has expressed her readiness to chair the review and begin work immediately if asked. The inquiry aims to understand what went catastrophically wrong at Leeds’ two maternity units.
